Membership Cancellation

By clicking "Proceed" I agree to the following terms of cancellation as outlined in my contract.

I understand if I have a 12 Month Agreement I am NOT eligible for cancellation and should request a billing suspension.
I understand that if I am eligible, my contract REQUIRES 45 DAYS WRITTEN NOTICE.
I understand I am responsible for all payments & fees until my cancellation is effective.
I understand if my billing date falls within 45 days of when I send in my request I will be billed again.
I understand my cancellation will not be processed if my account has a balance.

I understand that this form is a request to cancel my membership. Further, by clicking proceed, I acknowledge that I will be charged a Cancellation Fee of $40.00 per my membership agreement to be billed to the payment method on file. My membership will not be cancelled if I have a past due balance of any kind.

I understand that in order to complete the cancellation process I must provide the email receipt of this form to the gym during the gym staffed hours between Monday to Friday.
